Medical Records
The medical records department plays a key role in caring for our patients because it tracks every treatment, procedure, and diagnosis by physicians for each patient. At Loretto Hospital, our medical records staff works closely with attending physicians to ensure that the patient’s medical information is recorded accurately and in a timely manner. Additionally, because of the sensitivity of patient records, Loretto Hospital adheres to all federal HIPAA Privacy Rules, which protects the privacy of a person's health information, whether electronic, written, or verbal.
How to Obtain a Copy of Your Medical Record
In order to protect your privacy, we require written authorization and a valid photo ID to release your medical records. Below is the Authorization for Release of Medical Information. Please print and complete the form. Bring the completed form, along with a valid photo ID to the Medical Records Department at 645 S. Central Avenue, Chicago, Illinois 60644. You may also obtain a copy by contacting the Medical Records Department at (773) 854-1862.

Acceptable Forms of Identification
- State-issued ID Card (example: Driver’s License)
- Birth Certificate
- Social Security Card
- Passport
The Medical Records Department is available to assist you during regular business hours, Monday through Friday from 8:30 a.m. to 4:00 p.m.
When Will I Receive a Copy of My Medical Record?
Copies are processed within 7 to10 business days from the date the request is received but can take up to 30 days. The first copy of medical records for patients is free of charge. There may be a fee associated with each additional copy.
